Jan. 2016 - Aug. 2017Supporting expansion of evidence based health promotion programs targeting older adults and older adults with disabilities, primarily through strengthening collaboration among stakeholders at the state and local level. Worked on the initiative called “Healthy Aging NC” with various community and healthcare partners to establish an information/communication hub and website that centralized programs across the state. Programs included A Matter of Balance, Tai Chi for Arthritis, YMCA: Moving for Better Balance, and Otago Exercise Program. The communication hub or “Healthy Aging NC Resource Center” facilitates the coordination of evidence based activities in North Carolina with a goal of elevating the visibility of services. Included regional coalition building, project management, statewide technical assistance for programs, building cross referrals along a continuum of programs and with Chronic Disease Self Management programs, professional and community education on fall prevention, social media campaigning, strategic planning, and sustainability/business planning.
