Aug. 2022Conduct 6-8, 15-minute consult Screening Calls per day for High-Risk Clients assessing for safety, substance use, and eating disorders to determine the appropriate level of care and provider match. Utilize clinical knowledge and internal tools to curate an ideal provider match for clients based on clinical needs, schedules, and unique client preferences. Collaborate with cross-functional teams to pilot referral workflows to expedite care for clients referred via pre-existing referral relationships such as Stanford and UCLA health centers, resulting in new workflows and processes to be developed to improve patient experience and staff efficiency. Deliver comprehensive training programs for new hires, ensuring a smooth onboarding process. Remain “go-to” support to all new hires for ongoing questions, concerns, and case consultation. Actively participated in hiring panels, demonstrating expertise in candidate assessment by conducting thorough interviews, evaluating resumes, and providing valuable insights that contributed to the selection of 14 Senior Care Navigators. Respond to 80+ client and provider inquiries daily while utilizing EMR, EHR, and communication platforms including Welkin, Acuity, Zendesk, Telescope, Tableau and other internal tools to ensure new and existing clients are supported through their entire Octave life cycle. Represent the company at external events including community walks and employer benefits fairs to educate prospective clients about Octave’s services and offerings leading to 60+ new client leads. Serve as the representative for the Care Navigation Team on Octave’s Values Committee meeting monthly to discuss our new company values and how to integrate them into our culture leading to new initiatives such as our “Mixtape Sessions” to help build our culture and reinforce that “we are human beings first.”
