Spearheaded the implementation of social impact strategies to demonstrate the social return on investment for community, clinical and technological innovations. Supported the launch of the Innovation Bridge – a patient engagement site that promoted real-time access to personal clinical information and connections to community resources. Served as a key member of the Accountable Health Communities Model grant team – a five-year $4.5M grant funded by the Centers for Medicare and Medicaid Services. Led the recruitment of 289 nonprofit organizations to participate in cross-sector, socio-health collaborations. Established advisory boards for the Dallas Information Exchange Portal – a five-year, $13M grant funded by the W.W. Caruth, Jr. Foundation and Data Across Sectors for Health: Food for Health – a $200,000 program with the aim to alleviate hypertension, diabetes, and food insecurity in North Texas. Achieved 300% of the first year’s recruitment goal of nonprofit organizations within the first 6 months of employment. Achieved 100% of the software implementation goal within the grant cycle’s initial 9 months. Served as a co-presenter for the United Way of Metropolitan Dallas’ GroundFloor presentation on Pieces Catalyst, the Parkland Center for Clinical Innovation’s (PCCI) shared-savings initiative, which resulted in PCCI being selected as a GroundFloor Fellow and receiving $50,000 in funding for the shared-savings proof of concept. Project Manager for the Robert Wood Johnson Foundation’s Data Across Sectors for Health: Food for Health grant. Responsible for grant management and assisted with the recruitment of patients for the 22-month clinical study. Led community engagement efforts for PCCI and chartered community coalitions to alleviate diabetes, hypertension, food insecurity, and access to health care in disenfranchised communities.
