The RN Care Coordinator manages the continuing care needs of patients within DFCH/WH disease centers and is responsible for ensuring care that supports desired clinical, financial and process outcomes. The RN Care Coordinator utilizes patient assessment, care guidelines, knowledge of research protocols and anticipated outcomes, the full continuum of care resources, payer regulations, and communication to facilitate continuing care needs. Mobilizes resources to maximize efficiency of care delivery. Collaborates with clinical and administrative colleagues, Access Management and BWH-DFCI Care Coordination to achieve desired outcomes. He/she is a member of a multidisciplinary care team that requires effective communication and collaboration with co-workers and staff at the DFCI, BWH, and other collaborating institutions and agencies. Participates in the ongoing evaluation of practice patterns and systems while supporting efforts to improve quality, cost and satisfaction outcomes.
