Jan. 2013 - Dec. 2021Maintain a client caseload in my assigned area, plan personalized visits for each child based on their individual needs, schedule and provide in home specialized skills training with clients, supply parents with personalized case management information and skills for child to work on between home visits, provide support and resources for parents receiving a diagnosis for their child, communicate with parents and other staff members via phone, email, or text, coordinate and complete all applicable testing and paperwork as required by state laws, upload records, charts, and visit notes in a timely manner, collaborate with other therapists on cases and to schedule and complete joint testing visits, schedule, plan and teach group library classes in multiple county areas, complete and score initial and yearly qualification testing, request to add other services based on client scores and needs, review, maintain, and add additional IFSP goals to insure they remain appropriate for child, complete all necessary IFSP paperwork and grids, finalize time sheets, travel logs, and other monthly forms on time, attend staff meeting and program social events, maintain all required certifications, records, and trainings.
