Sep. 2020Brisbane, Queensland, Australia•Maintain positive relations working in partnership with clients/families, and other stakeholders both external and internal, to plan, develop and provide a person-centered holistic approach within legislative and organisational guidelines
•Assist clients to maintain their optimum degree of independence, dignity, privacy, choice, and quality of life.
•Ensure clients are informed of their rights and responsibilities
•Liaise closely with Care Coordinators (CHSP/HCP) to ensure a smooth handover of clients once on-boarded
• Coordinate the filling of shifts in a financially responsible manner. This includes:
• Rostering additional shifts and shift alterations requested by clients in keeping with basic funding guidelines and
• Minimizing the use of overtime and external agencies
Be a focal point for client and staff contact and provide clear and supportive communication when:
• Acting as the contact for After Hours service from clients & staff;
• Interpreting service summary information and relaying this to staff;
• Resolving the immediate service issues of clients and staff including situations involving distressed staff and clients;
• Communicating with staff and clients directly regarding shift filling.
Set up and maintain support data in the rostering system for both employees and clients including preferences, availability, contact details, client medical needs and other data as required.
• Undertakes other duties as required and guided by the Rostering Facilitator
