DotHouse HealthLead Family Partner/ Family Support Coordinator
Aug. 2011 - Aug. 2016Dorchester, MAAs a Family Partner, I provide social support services to families with young children. This experience involves skills of managing community outreach programs and appreciation of the fact that the support services are provided according to the individual needs of each person. Moreover, I provide trauma informed care for families who have young children who are at risk or have social and emotional developmental concerns. I help caregivers brainstorm goals they want to work on, enhancing their self-efficacy skills and teaching them skills on how to be an advocate for their family's needs. Continuous education through processes, such as training of other staff members, improves the quality of services offered. I supervise and train staff members on behavioral needs of young children and families, particularly in the early child education centers. Through combined efforts and cooperation among the social workers and the communities, we have been able to provide services depending on the needs of the client and the prevailing environment. As the Family Support Coordinator (FSC), I oversee and provide supervision to all staff in our Case Management department; Responsible for the day to day operations. I provide mentorship and support to the Behavioral Health Integration project and the BPHC Trauma Grant at Dothouse Health. I am responsible for monitoring and developing triage porotocols for patient care coordination, grant reporting activities and data reports as well as providing support to the project goals to increase access to mental health services. I coordinate and co-facilitate the DotHouse Heath Family Fun nights, the Parenting Journey groups and other outreach events.
