Jennifer's team consisted of physician leaders, biostatisticians, project managers, implementation specialists, clinical improvement coordinators and health data analysts. The HVHC team helped implement quality improvement projects focused on patient engagement, shared decision making and care management in over 180 clinics across the US in 2013. The High Value Healthcare Collaborative (HVHC) was a consortium of 20 healthcare delivery systems and The Dartmouth Institute for Health Policy and Clinical Practice. The mission of the HVHC was to improve healthcare value – defined as quality and outcomes divided by costs, across time – for its service population, in a sustainable manner, while serving as a model for national healthcare reform. HVHC members shared data and compared performance to identify, test, and disseminate best practice care models and payment models. HVHC health systems included: Baylor Scott & White, Beth Israel Deaconess Medical Center, Dartmouth Hitchcock, Denver Health, EMHS, Hawai'i Pacific, Intermountain, Mayo Clinic, MaineHealth, NYU Langone, North Shore LIJ, Providence, Sinai Health, UCLA, University of Iowa and Virginia Mason.
