Coordination of Care- Coordinate and monitor patient care plans across different healthcare providers and services. - Facilitate communication between patients, doctors, nurses, and other healthcare professionals. Ensure that all necessary referrals and authorizations are completed. Administrative Duties- Maintain and update patient records, ensuring accuracy and confidentiality. - Handle billing inquiries and assist patients with understanding their insurance coverage. - Prepare and manage documentation required for patient care, such as consent forms and treatment plans. Patient Advocacy - Advocate for patientsโ needs and ensure they receive the appropriate care. - Help patients navigate the healthcare system, including insurance and financial issues. - Assists in resolving any barriers to care, such as transportation or language issues. Quality Assurance- Monitor patient outcomes and satisfaction to ensure high-quality care. - Implement and follow up on quality improvement initiatives. - Participate in meetings and contribute to the development of patient care policies and procedures. Support for Clinical Staff- Assist healthcare providers with patient assessments and care planning.
