Apr. 2018 - Dec. 2018Coos Bay, ORI was continually asked by leadership to take on more responsibilities while taking lead point in coordinating public health research, thought leadership, reporting, programming, grant writing, and information dissemination. My abilities were showcased after I was tapped by the executive team to play a critical part in the clinic's value-based transformation initiative. As part of my work, I completed a series of population assessments that were recognized as "a game changer for clinicians and patients" by the Chief Financial Officer (CFO)/Chief Operating Officer (COO). Utilizing the data I gathered from the Quantum Geographic Information System (QGIS), I successfully identified pockets of disease prevalence in regional communities. I then authored a series of key reports that were instrumental in assisting the clinic with developing data-driven plans that significantly enhanced care across the entire region. Among my other major initiatives, I… Created in-depth risk analysis reports that highlighted patient segments that were overutilizing ER and other clinic resources. Designed and rolled out data-driven programs that were critical in helping the clinic prepare for Comprehensive Primary Care Plus (CPC+), a five-year, multi-payer initiative to improve primary care in the US. Crafted presentations for a host of public health programs on topics such as flu vaccines and employed various other methods to communicate vital public health information (Social Media, Mailings, etc.).
