Agincare Care Coordinator
Apr. 2023 - Dec. 2023Witney, England, United Kingdom• Assess the needs and preferences of clients to develop comprehensive care plans and evaluate clients' health conditions, functional abilities, and social support systems to create individualized care plans that address their specific needs.
• Act as a liaison between clients, healthcare professionals, and other service providers to ensure seamless coordination of care and schedule and organize home visits, medical appointments, therapy sessions.
• Advocate for the clients, ensuring they receive appropriate services and support and communicate and collaborate with healthcare professionals, social workers, and community resources to ensure clients' needs are met.
• Regularly monitor clients' progress and evaluate the effectiveness of care plans and review the outcomes of interventions.
• Provide health education to clients and their families, promoting self-management and disease prevention strategies and offer guidance on medication management, nutrition, exercise, and other aspects of maintaining good health.
• Maintain accurate and up-to-date records of clients' assessments, care plans, interventions, and progress and ensure compliance with legal and regulatory requirements, complete necessary documentation, and prepare reports as required by public health agencies or finding bodies.
• Collaborate with a multidisciplinary team, including physicians, nurses, therapists, social workers, and other healthcare professionals.
• Ensure a smooth transition of care between different healthcare settings and facilitate the discharge process from hospitals or rehabilitation centres and arrange for home-based care services and coordinate with community resources to support clients' long-term needs and link them to appropriate social services.
• Contribute to quality improvement initiatives within the domiciliary care organization and identify opportunities for improvement, participate in performance evaluation processes,
