Feb. 2019 - Mar. 2020Auckland, New ZealandPatient Education and Support: My primary responsibility included providing education and support to patients living with Multiple Sclerosis and involved assessing patients' needs, providing information and resources about the disease, explaining treatment options, and helping patients understand and manage their symptoms. I would also offer emotional support, facilitate self-management strategies, and provide guidance on lifestyle modifications to promote overall well-being. Conducting Face-to-Face Appointments: I am experienced in conducting face-to-face appointments with patients, and I understand the importance of building rapport, establishing trust, and providing personalized care. I would conduct comprehensive assessments of patients' physical, emotional, and social needs, develop care plans, and monitor patients' progress over time. I am skilled in using clinical assessment tools, documenting patient interactions, and ensuring confidentiality and privacy. Liaising with Health and Disability Services: I have experience in liaising with health and disability services in the health sector and community to provide appropriate support to patients with Multiple Sclerosis. This included collaborating with healthcare providers, coordinating care plans, advocating for patients' needs, and facilitating referrals to other services or specialists. I am adept at building professional relationships, communicating effectively with interdisciplinary teams, and navigating the healthcare system. Providing Health Education to the Community: In addition to supporting patients with Multiple Sclerosis, I would also play a role in providing health education to the community. My role involved conducting educational sessions, workshops and support groups for patients, caregivers, and other stakeholders. I would raise awareness about Multiple Sclerosis, promote health literacy, and provide education on prevention, management, and coping strategies for patients and their families.
