Worked primarily in unit dose where I approved physician orders through EPIC to dispense oral and IV medications to be tubed to the floors. Additionally, this is where I approved repackaged medications, checked in controlled substance orders, and verified crash cart contents for code use.
Experience approving specialty medications used in chemo, dialysis, inpatient floors, and outpatient infusion therapy. Furthermore, I was able to help providers with dosing and adjusting IVIG for multiple acute flare-ups or chronic autoimmune conditions.
Assisted physicians with dosing requests, infectious disease recommendations, and order entry. Moreover, nurses were also assisted with medication administration questions and IV compatibility.
Quickly responded to preparation of STAT interventional medications needed for Stroke (Example: TNK), Neuro lab, Cath lab, Emergency Room/Traumas, or Operating Rooms.
Dosed pharmacokinetic consults for all aminoglycosides and Vancomycin. Additionally, I utilized Insight Rx to dose Vancomycin to decrease renal toxicity and reach therapeutic AUC faster.
Participated in dosing antibiotics and anticoagulants according to renal function, weight, BMI, and age. Additionally, I regularly adjusted these medications for patients requiring hemodialysis, peritoneal dialysis, or sustained-low efficiency dialysis (SLED).
Reviewed patient’s medications per consults for QT prolonging medications to facilitate the physicians discontinuing, holding, or changing therapies to avoid future arrhythmic interactions. Entered and adjusted TPNs in Abacus to appropriate concentrations safe to be compounded and administered to NICU babies, pediatrics, inpatient adults, and LTACH patients.
