case discussion and performance reviews.
• Hire and evaluate a team of Care Navigators and Outreach Workers under his/her own
supervision.
• Meeting performance goals set by APICHA’s Client Services Department.
• Responsible for creation and implementation of service plan, supported by Care Navigator.
• Coordinates and oversees the implementation of the comprehensive care plan.
• Coordinates and oversees services between patient and extended care team providers to ensure
that integrated care plan is fully implemented.
• Provides clinic-based health education in conjunction with regularly schedules primary care visits
as needed.
• Performs the initial entitlements assessment and collates the comprehensive plan.
• Works closely with the interdisciplinary care team including PCP, psychiatrist, therapist, ACT
Team, etc.
• Provides backup to Care Navigators as needed.
• Collate data reports (e.g., appointment dispositions, laboratory results, etc.) produced by clinic
information systems (EMR).
• Maintain client-related records and other required documentation according to the protocols and
standards of APICHA’s Client Services Department.
• Working as a part of the unit at the quality assurance and continue quality improvement (CQI)
tasks.
• Represent the agency at various venues to promote agency services.
